If you would like a copy of your own Massachusetts criminal record, complete this form, sign it in front of a notary public, and mail it, along with a check or money order made payable to the Commonwealth of Massachusetts in the amount of $25.00 and a self-addressed stamped envelope to:

Walk-in service is not available. You must properly complete the CORI Request Form and have it notarized. You will receive a copy of your record in the mail.
You are also entitled to request and obtain a copy of your Massachusetts Driving Record.
The both the Registry of Motor Vehicles and the Board of Appeal have access to your criminal record and they will review it during any hardship license hearing.
